Strive Health & Prominence Health Plan Create Kidney Care Partnership For Nevada Medicare Advantage Members

Strive Health (Strive), which provides value-based kidney care arrangements partnered with commercial/Medicare health coverage entity Prominence Health Plan (Prominence) launched a partnership to improve kidney care in Nevada. The services are for Prominence Health Medicare Advantage plan members with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). More than 2,000 members with either CKD or ESKD are eligible to participate in this program annually.

Through a shared savings arrangement, Strive Health will work closely with Sierra Nevada Specialty Care, a Nevada-based nephrology clinic, to coordinate care for Prominence Health Medicare Advantage members. Sierra Nevada Specialty Care will receive fee for service payments during the contract year. Shared savings is a payment strategy that offers incentives for provider organizations to reduce health care spending for a defined consumer population by offering them a percentage of net savings realized as a result of their efforts.

Strive’s health care professionals will work closely with primary care physicians, nephrologists, and transplant specialists in the Prominence and Sierra Nevada Specialty Care networks to ensure that health care consumers have a seamless care journey. A specialized care team will provide 24-hour access to care, including telehealth visits and care coordination. The goal is to deliver a clinical care model that helps slow the progression of kidney issues and preserve kidney function for as long as possible.

The Strive Kidney Heroes™ interdisciplinary clinical care team, which includes nurse practitioners, dietitians, care coordinators, and licensed clinical social workers, use Strive’s technology platform to gather data from various sources and gain a better understanding about the consumer’s risk of hospitalization or the progression of their disease. The care team offers support through an individual’s kidney-care journey and serves as an extension of their physician to help manage both kidney disease and comorbid conditions (such as diabetes). The result is less frequent hospitalizations, fewer readmissions, lower costs, and better outcomes.
