Strive Health & Evolent Care Partners Create Kidney Care Collaboration

On April 12, 2022, Strive Health and Evolent Care Partners (ECP) announced the launch of a kidney care initiative for primary care professionals (PCPs) in ECP’s Shared Savings Program accountable care organization (ACO) in the Michigan and North Carolina markets. The initiative will help the PCPs manage care for Medicare beneficiaries attributed to the ACO who have advanced stages of chronic kidney disease (CKD) and end stage kidney disease (ESKD). Through the arrangement, over 200 PCPs participating in ECP’s ACO will collaborate with Strive Health to identify beneficiaries at high risk for kidney disease as early as possible, and provide tailored interventions to slow progression of advanced CKD and prevent hospitalization.

Strive Health will provide the PCPs with access to data and technology tools, administrative support, management expertise, and an interdisciplinary clinical care team that includes highly specialized care coordinators, nurse practitioners, dietitians, pharmacists, and licensed clinical social workers. The participating professionals manage care for up to 1,200 Medicare fee-for-service beneficiaries living with advanced CKD and ESKD who are in the Michigan and North Carolina ACO markets .

Strive Health will use machine learning and predictive analytics to determine risk factors for beneficiaries with CKD at stages 4 and 5, and ESKD through analysis of consumer-specific data from the ECP’s ACO. Beneficiaries identified through this process will be referred to Strive Health. Strive Health will collaborate with nephrologists to provide these beneficiaries with specialized kidney care. For beneficiaries with CKD at stage 3, ECP will work with Strive Health to review disease progression and design a care plan.

Through partnerships with medical groups, nephrologists, health systems, and payers, Strive Health manages over 44,000 individuals with CKD and ESKD. Strive Health’s core solutions include population health, Strive Care Partners™ (a value-based nephrology platform) and complete dialysis. Strive Health forms an integrated care delivery system using a combination of high-touch care teams, predictive analytics, advanced technology, seamless integration with local professionals and next-generation dialysis services.

ECP’s ACO is composed of more than 1,000 independent primary care physicians located across nine states. It provides physician participants with resources needed to participate in the Medicare Shared Savings Program (MSSP) and succeed within two-sided performance-based contracts. Participating physicians in ECP’s ACO utilize ECP population health tools and care coordination services to care for more than 90,000 Medicare beneficiaries.
