This OPEN MINDS Executive Roundtable was delivered on December 14, 2023 by with Timi Leslie, President, BluePath Health, and Anwar Zoueihid, Vice President of Long-Term Care Services & Supports, Partners in Care Foundation, who presented an overview and key elements of California’s Health and Human Services Data Exchange Framework, as well as available funding opportunities to assist organizations with infrastructure development. Richard Louis, III, OPEN MINDS Vice President West Region, moderated the session.
The California Advancing and Innovating Medi-Cal (CalAIM) initiative was launched in 2022, and from now through 2027, the Medi-Cal delivery system will transition to using a population health approach, prioritizing prevention and whole-person care. Critical to this transformation is the implementation of new data-sharing approaches. Beginning in 2024, a first-ever, statewide data sharing agreement—California’s Health and Human Services Data Exchange Framework—will accelerate and expand the exchange of health information among health care entities, government agencies, and social service programs.
