This presentation occurred at The 2024 OPEN MINDS Institute For Operational Excellence during The 2024 OPEN MINDS Whole Person Care Summit. Attendees heard from Dr. Jacqueline Webster, LAC, NCC, Assistant Director, Population Health, and Derrick Baker, Quality Improvement Manager, Copa Health, as they discussed the importance of understanding population needs in supporting whole person care and value-based reimbursement, methodologies and tools for tracking risk indicators and managing chronic health conditions, plus strategies for reporting this data to health plans to negotiate higher reimbursement rates and strategies for analyzing data to measure, modify, and improve whole person care programs over time.
