On September 15, 2022, the California Department of Health Care Services (DHCS) published this California Advancing and Innovating Medi-Cal (CalAIM) initiative population health management (PHM) policy guide. Thise is one of three key DHCS guidance documents that set forth comprehensive requirements applicable for all Medi-Cal Managed Care health plans (MCPs) for the implementation of PHM, beginning on January 1, 2023. The PHM Program is designed to ensure that all members have access to a comprehensive set of services based on their needs and preferences across the continuum of care, which leads to longer, healthier, and happier lives, improved outcomes, and health equity
