Signify Health added spirometry testing to detect chronic obstructive pulmonary disease (COPD) to its in-home diagnostic and preventive services offering for Medicare Advantage and Medicaid plan members. The company offers a health care analytics platform, technology and nationwide networks to create and power value-based payment programs. It offers in-home diagnostic testing for several chronic health conditions to reduce barriers to timely diagnosis and treatment decisions by individuals and their physicians.
In addition to detecting COPD, Signify Health’s Diagnostic and Preventive Services line supports early detection, diagnosis, and management for peripheral arterial disease, colorectal cancer, chronic kidney disease, diabetes, and low bone density. These diagnostic and preventive services are performed during the Signify Health In-Home Health Evaluation to deliver a comprehensive and convenient care experience for consumers.
Signify Health uses advanced analytics to identify health plan members with gaps in care and to connect them with the appropriate diagnostic and preventive health services. Delivering these services in the home reduces barriers to care by providing health plan members with convenient access to high-value care to improve health outcomes. Its solutions include a holistic suite of clinical, social, and behavioral services to address an individual’s health care needs and prevent adverse events that drive excess cost, all while shifting services towards the home.
In 2022, Signify Health’s clinicians expect to connect with nearly 2.5 million unique members in the home, both in-person and virtually. The Signify Health solutions are intended to support value-based payment programs by aligning financial incentives around outcomes, providing tools to health plans and healthcare organizations designed to assess and manage risk and identify actionable opportunities for improved patient outcomes, coordination and cost-savings.
In September 2022, CVS Health entered a definitive agreement to acquire Signify Health for about $8 billion. CVS Health and Signify Health anticipate that the transaction will close in the first half of 2023. In March 2022, Signify Health acquired Caravan Health, which expanded its focus on value-based care and population health.
