The New York Department of Health (DOH) awarded nine organizations grants totaling $500 million over three years to create new regional Medicaid Social Care Network (SCN) programs. The SCNs are part of the state’s Medicaid Section 1115 Demonstration Waiver. The SCNs will integrate health care and health-related social care needs (HRSNs), such as nutrition, housing supports, transportation, and case management for eligible Medicaid members. The goal is to address health disparities and improve population health.
The nine organizations and their SCN regions are as follows:
- Care Compass Collaborative (Southern Tier)
- Finger Lakes IPA (Finger Lakes)
- Health & Welfare Council of Long Island (Long Island)
- Healthy Alliance Foundation (Capital Region, Central New York, North Country)
- Hudson Valley Care Coalition (Hudson Valley)
- Public Health Solutions (Manhattan, Queens, Brooklyn)
- SOMOS Healthcare Providers (Bronx)
- Staten Island Performing Provider System (Staten Island)
- Western New York Integrated Care Collaborative (Western New York)
The regional SCNs will be responsible for developing a network of community-based organizations (CBOs) to deliver services. Within their regions, the health care professionals, behavioral health professionals, and care managers will refer members to the SCN for their region to screen Medicaid beneficiaries, issue referrals as needed to the network, and provide case management for those beneficiaries receiving HRSNs. DOH intends to develop a fee schedule tailored to each region that will be used to reimburse the CBOs for services provided to Medicaid managed care organization (MCO) members.
Five of the SCNs are partnering with Unite Us to provide the infrastructure for collaboration in each region’s network of CBOs, health care provider organizations, and MCOs. The five SCNs are Care Compass Collaborative (CCC), Health and Welfare Council of Long Island (HWCLI), Healthy Alliance Foundation Inc., Hudson Valley Care Coalition Inc., and Public Health Solutions (PHS). These SCNs will represent 72% of the state’s Medicaid beneficiaries.
