The MENTOR Network, a national provider of home and community-based specialized health care, has rebranded as Sevita. The new branding is intended to represent the organization’s focus on using new models of care, innovation, technology, and collaboration across the organization. The organization has entered a joint venture with Matt Holder, M.D., MBA; and Henry Hood, DMD, of Lee Specialty Clinic to launch virtual clinics to provide medical care to people with intellectual/developmental disabilities (I/DD) nationwide.
The new clinics intend to leverage virtual capabilities tied to a custom built, I/DD-focused electronic health record (EHR) allowing clinical staff to meet consumers where they are most comfortable, while expanding access to broader geographies. The clinics will also provide training/education for clinical professionals and staff, engagement with the I/DD community, and population health efforts.
Founded in 1967, the organization provides home and community-based services for more than 50,000 adults, children, and their families across 40 states. Sevita’s service line includes innovative and individualized supports to address the physical, intellectual, or behavioral challenges people face. It serves people with I/DD, people with complex care needs, people recovering from brain and spinal cord injury, seniors in need of daily support, children in foster care, adults and children with autism, and other individuals who may require care across the lifespan.
