Northwest Primary Care (NWPC) partnered with CareSignal, which provides a remote consumer monitoring platform, to manage vulnerable consumer populations and improve NWPCs performance in at-risk contracts. The partnership expands NWPCâs consumer engagement capabilities by engaging previously unmanaged rising-risk populations with Deviceless Remote Patient MonitoringÂź. Using CareSignalâs technology, NWPC is now able to monitor the whole health of patients across 30 high-cost chronic and behavioral health conditions, and is currently prioritizing chronic obstructive pulmonary disease, congestive heart failure, hypertension, diabetes, and depression.
With CareSignalâs automated platform, NWPC can offer care management and preventative care to the rising-risk population allowing it to succeed in various at-risk reimbursement models, including CPC+, Medicare Advantage, and at-risk contracts. By collecting real-time consumer symptoms through convenient text and phone-based communication, CareSignalâs platform enables care teams to reduce unnecessary hospitalizations and readmissions. The partnership brings NWPC new digital solutions for both chronic and behavioral health use-cases.
Since implementing CareSignal in October 2021, NWPC has engaged hundreds of consumers and provided early identification of those in need of support, resulting in 34 emergency department visits averted. It engaged 576 consumers and identified 449 acute escalations of health conditions. In total, the platform collected 22,800 automated consumer touchpoints.
