Bon Secours Mercy Health in Ohio and Strive Health launched a new collaborative relationship to deliver care to people with kidney disease. Strive Health will deliver a new clinical model for nearly 8,000 Bon Secours Mercy Health consumers with chronic kidney disease (CKD) or end-stage kidney disease (ESKD).
For this collaboration, Bon Secours Mercy Health will use Striveās proprietary technology platform, Strive Kidney Heroes⢠interdisciplinary clinical care teams, and Striveās CareMultiplier⢠technology.
The Strive clinical care team includes nurse practitioners, dietitians, pharmacists, care coordinators and licensed clinical social workers. These team members serve as an extension of the consumerās physician to help manage both kidney disease and comorbid conditions, such as diabetes, that can impact a consumerās overall health.
The CareMultiplier gathers data from hundreds of sources to gain a holistic view of the consumerās experience. That information illustrates the consumerās risk of hospitalization or progression of disease, enabling provider organizations to better manage care based on each individualās needs.
Strive’s core solutions include Population Health, Strive Care Partners⢠(a value-based nephrology platform) and Complete Dialysis. The company manages over 50,000 patients with CKD and ESKD through partnerships with providers and payors. Initially launched in partnership with SSM Health, Strive is the leading value-based kidney care company currently working with health systems. Strive partners with commercial and Medicare Advantage payors, Medicare, health systems and physicians through flexible value-based payment arrangements, including risk-based programs. Striveās case management and population health programs are accredited by the National Committee for Quality Assurance (NCQA), and its technology platform, CareMultiplierā¢, is certified by HITRUST.
Bon Secours Mercy Health is one of the 20 largest health systems in the United States and the fifth-largest Catholic health system in the country.
