California Medicaid health maintenance organization Health Plan of San Joaquin (HPSJ), which has more than 450,000 members, is set to use ZeOmega’s Jiva platform to manage health care for members of Medi-Cal, California’s Medicaid program. The platform will help HPSJ support a whole-person approach to care by using Jiva’s tools for utilization management, care management, appeals and grievances, and social determinants of health.
HPSJ said it expects the Jiva platform to assist in its efforts to improve quality of care, participate in value-based care, monitor consumer outcomes, consumer engagement, and implement health equity. The platform will also help HPSJ share health care data and manage health care costs for Medi-Cal members.
Medicaid population-focused organizations around the U.S. use ZeOmega for population health management. HPSJ is ZeOmega’s fifth Medi-Cal plan partnership, following Kern Health Systems, CalOptima Health, San Francisco Health Plan, and Central California Alliance for Health. ZeOmega is based in Texas and was founded in 2001.
