Achieving consistent quality outcomes in value-based care isn’t about working harder, it’s about building systems and culture that make doing the right thing the path of least resistance. In this session, two network leaders from Alera Health’s ONEcare family share hard-won strategies for closing care gaps and driving HEDIS performance across clinically integrated networks serving some of the most complex populations in pediatric primary care and behavioral and mental health care.
You’ll hear practical, proven best practices for educating staff, redesigning workflows, and using data to motivate action, including how to identify care gaps before appointments, create talk tracks that help providers address sensitive topics with confidence, establish clear accountability across multi-agency networks, and foster cooperation among agencies that are otherwise competitors.
The session will also take a focused look at applying these strategies in pediatric and behavioral health settings, drawing on specific examples from Children First of North Carolina (CFNC), including child-focused HEDIS measures and what it takes to make quality improvement sustainable in a youth-serving network.

Nancy Jo Sutton-McLeod
Senior Practice & Network Transformation Specialist, Alera Health
Nancy Jo Sutton-McLeod is a licensed clinical social worker with over 20 years of experience spanning some of the most complex and diverse populations in behavioral health, from crime victims and therapeutic foster children to soldiers, geriatric patients, and individuals in recovery. An Arizona native, she brings a deep commitment to her home state and the communities she serves.
In her current role at Alera Health, Nancy Jo manages quality and operations for two Arizona-based ONEcare clinically integrated networks. Over the past year and a half, she has become a driving force in practice and network transformation, managing value-based contracts and leveraging data analysis to design and implement strategies that move the needle on HEDIS outcomes across both networks.

Trish Smith
Practice & Network Transformation Specialist II, Alera Health
Trish Smith is a public health and healthcare quality professional with a clinical background as a licensed practical nurse and a Master of Public Health degree. Her experience spans over 20 years in pediatric care for individuals with intellectual and developmental disabilities, care management, maternal-child health, and healthcare quality improvement. She has also worked as an International Board-Certified Lactation Consultant, bringing a strong foundation in family-centered care and health education.
In her current role at Alera Health, Trish supports pediatric practices in navigating quality, compliance, and performance improvement while keeping the people behind the metrics at the center of the work. She helps practices strengthen workflows, use data more effectively, and implement sustainable strategies that support value-based care and improved outcomes.
