Insurers GuideWell & Highmark Invest In Healthmap Kidney Disease Management Solution

|

By OPEN MINDS Circle

On May 2, 2022, Healthmap Solutions, which provides a population health program focused on kidney care management, announced that the investment arms of health insurers GuideWell and Highmark led a $35 million investment round. GuideWell and Highmark Inc., are both Healthmap customers. With the new funding, Healthmap intends to expand to new states, but has not announced which ones. It anticipates that by the end of 2022, it will have about 150,000 members under contract across its commercial, Medicare, and Medicaid lines of business. Healthmap’s program features advanced technology, predictive analytics, and clinical expertise to detect early kidney disease and recommend interventions to delay or slow disease progression.

GuideWell is the parent of Florida Blue and Triple-S Management, the Blue Cross Blue Shield plans of Florida and Puerto Rico. Florida Blue Medicare has contracted with Healthmap since the first half of 2021.

Highmark Ventures is the corporate venture arm of Highmark, Inc., a non-profit health care company and integrated delivery network that offers Blue Cross Blue Shield branded plans in Pennsylvania, Delaware, West Virginia, and New York. On March 2, 2022, Healthmap announced a partnership with Highmark Health (Highmark). This partnership introduced Healthmap’s Kidney Health Management (KHM) program to Highmark members living with chronic kidney disease (CKD) and end stage renal disease (ESRD).

Healthmap was founded in 2013. It is accredited by the National Committee for Quality Assurance (NCQA) as a Kidney Population Health Management company. It serves health plans and provider organizations seeking value-based solutions that improve the clinical care and financial performance of their programs for high-risk, high-cost individuals with kidney disease. It partners with primary care physicians, nephrologists, cardiologists, endocrinologists, pharmacists, dialysis provider organizations, and other critical kidney care specialties.

Using its advanced technology, predictive analytics, and clinical expertise Healthmap detects kidney disease early and recommends clinically proven interventions to delay or slow disease progression. Healthmap’s Care Navigation team works with consumers and a full spectrum of provider organizations (primary care, nephrologists, cardiologists, endocrinologists, and dialysis providers), to deliver personalized whole-person care. Healthmap’s approach is to proactively plan care transitions, and optimize renal replacement therapy, with a focus on in-home dialysis treatment and transplant. The goal is to improve care, outcomes, and the experience of care for the person living with kidney disease, while lowering total medical costs for health plans and provider groups.
