Bon Secours Mercy Health Ohio Partners With Strive Health On Kidney Care Initiative

On May 17, 2022, Strive Health and Bon Secours Mercy Health in Ohio launched a new collaborative relationship to deliver care to people with kidney disease. Strive Health will deliver a new clinical model for nearly 8,000 Bon Secours Mercy Health consumers with chronic kidney disease (CKD) or end-stage kidney disease (ESKD). For this collaboration, Bon Secours Mercy Health will use Strive’s proprietary CareMultiplierā„¢ technology and the Strive Kidney Heroesā„¢ interdisciplinary clinical care team.

The new clinical model provides additional, specialized clinical services to Bon Secours Mercy Health’s consumers with CKD or ESKD as an extension of the care already provided by the Bon Secours Mercy Health care team and physicians. Strive Health’s Jackson Brasher, Vice President of Health Systems Business Development, said ā€œThe goal is to move upstream in the kidney disease journey and help consumers better understand and manage their conditions to slow progression of disease.ā€ Strive’s CareMultiplierā„¢ technology gathers data from hundreds of sources to gain a holistic view of the consumer’s experience. That information illustrates the consumer’s risk of hospitalization or progression of disease, so that provider organizations can better manage care based on each individual’s needs.

The Strive clinical care team includes nurse practitioners, dietitians, pharmacists, care coordinators and licensed clinical social workers. These team members serve as an extension of the consumer’s physician to help manage both kidney disease and comorbid conditions, such as diabetes, that can impact a consumer’s overall health.

The care model combines technology-enabled earlier identification of CKD coupled with robust clinical support services to address each consumer’s individual, whole-person care needs. Strive will help Bon Secours Mercy Health consumers with CKD navigate transplant programs, select kidney replacement therapies, and increase home dialysis utilization. Strive will also help consumers manage their other medical conditions. Strive can provide services in-person, via video conferencing, or via telephone.
