Point32Health, the parent company of Harvard Pilgrim Health Care and Tufts Health Plan, is collaborating with Monogram Health in an at-risk arrangement to provide kidney care management and treatment services for Tufts Health Plan and Harvard Pilgrim Health Care commercial members. Monogram is a specialty organization that provides in-home nephrology, primary care, and benefit management services for individuals with chronic kidney and end-stage renal disease. A key goal is providing early-stage in-home services and slowing disease progression.
In this collaboration, Monogram Health will assume full financial risk for Point32Health’s members with kidney disease. It will work to increase access to early in-home access to kidney care and to benefit management services. Additionally, Monogram will participate in research with the Harvard Pilgrim Health Care Institute’s Department of Population Medicine. This department is the result of a collaboration between Harvard Pilgrim Health Care and Harvard Medical School focused on innovative teaching and research to address health care issues. Point32Health and Monogram Health are exploring opportunities to expand the program across other lines of Point32Health’s business portfolio.
Point32Health is a leading health and wellbeing organization. Through its founding organizations Tufts Health Plan and Harvard Pilgrim Health Care, Point32Health seeks to improve population health. It currently serves 2.2 million members across New England. Point32Health has not reported how many of its members meet the clinical qualifications to be referred to Monogram. Point32Health did not disclose the payment model for Monogram’s services, other than describing the risk arrangement as full risk.
Monogram provides in-home nephrology, primary care, and benefit management services for individuals with chronic kidney and end-stage renal disease. It has value-based partnerships with leading health plans and provider organizations across 33 states and all insurance products. Monogram is privately held by Frist Cressey Ventures, Norwest Venture Partners, and TPG Capital.
