About
The ONEcareāÆPopulation Health Academy is designed specifically for behavioral health executives, managers, and clinical leadership to increase awareness and understanding of population health best practices, value-based contracting, and integrated systems of care. The ONEcare Population Health Academy is sponsored by Alera Health in partnership with OPEN MINDS.
Participants will learn from population health experts the purpose and promise of population health, data aggregation and analytics, clinical quality improvement, elements of successful value-based contracting, total cost of care management, building a clinically integrated system of care, and more! While 10% of the population expends 70% of healthcare resources, over half of that same population experience a behavioral health conditionāwith the majority having 2+ chronic conditions.
The fastest growing value-based arrangements are focused on whole-person care for these patients with a primary or secondary behavioral health condition. The ONEcare Population Health Academy will provide executives and management with education and skills to effectively navigate a changing payment and care delivery landscape. The Academy offers opportunities to interact directly with industry experts. The Academy is ad-free, offering both virtual and face-to-face options to accommodate busy executives. In addition, Academy participants will have access to curated information, webinars, institute-based knowledge partner sessions, and the Whole Person Summit.
The Academy teachings are complimented by the vast array of content available in the OPEN MINDS platform, including access to an online value-based readiness assessment and a support line for immediate help in accessing tools and information. The newsletter will highlight topics specific to trends, policies, and best practices for integrated systems of care, population health, and value-based care. Academy Participants will receive an invitation to the invitation-only ONEcare Symposium held each June in New Orleans.
