Alera Health specializes in building Integrated Systems of Care (ISOCs), called ONEcare, designed to promote insight and communication between providers of primary, behavioral health, hospital, and social support agencies and to drive value-based arrangements.
One of the new offerings through the premier partnership is the ONEcare Population Health Academy. The “Academy” is designed to orient executives, managers, and clinicians on various population health management topics such as data aggregation and analytics, value-based contracting, care coordination technologies, and building clinically integrated systems of care. The Academy is especially designed for behavioral health providers who are taking the lead with whole person care for complex needs populations and seats up to 500 individuals.
